GAFC, or Group Adult Foster Care, does not provide skilled nursing care. Instead, it focuses on assisting with activities of daily living, such as bathing, dressing, meal preparation, and medication reminders. If you require ongoing medical or nursing services, you may need to explore other care options in addition to GAFC.

To apply for GAFC, you should first contact our office to determine your eligibility and discuss your care needs. Our team will guide you through the application process, which includes completing necessary forms and providing documentation of income and medical requirements. Once your application is submitted, we will work with you to develop a personalized care plan that best fits your situation.

If your needs change overtime, you should notify our office as soon as possible. We will reassess your situation and adjust your care plan to ensure you continue receiving the appropriate level of support.
