Services Covered Under GAFC Program

GAFC Program Services Include:

Personal Care

Assistance with bathing, dressing, grooming, and toileting.

Homemaking

Help with housekeeping, laundry, and meal preparation.

Medication Reminders

Ensuring that medications are taken as prescribed.

Care Management

Ongoing care coordination and health monitoring.

Recreational Activities

Enjoy companionship and fun through group game.

Group Adult Foster Care (GAFC)

Group Adult Foster Care (GAFC) is a Personal Care Assistance Program funded by MassHealth. Its purpose is to help individuals remain safe at home and avoid hospital or nursing home stays. There are no out-of-pocket costs — MassHealth covers all expenses. What makes us different is our focus on building trust, relationships, and a supportive environment where each person feels valued. Beyond basic care, we strive to enhance the emotional, social, and physical well-being of every member we serve.

Eligibility Criteria

Must be 22 years or older
Must live in subsidized housing
Must have a primary care physician
Must meet MassHealth income requirements
Must require help with at least two ADLs (Activities of Daily Living)
